October 2011 -- Stop ALD update on driving forward research on faster therapies for children in immediate need and research on anti-oxidant effectiveness on AMN symptoms.

Dear Stop ALD Foundation Supporter,

To date, the Foundation has played a key role in finding a therapy for ALD patients whose disease has just started to progress. Looking forward to the fourth quarter of 2011, we will turn our attention to two different and, for us, very significant goals:  (1) Driving forward a therapeutic approach for patients whose disease has progressed beyond the ability of current therapies to be effective, and (2) determining whether anti-oxidant treatments are of benefit to help treat AMN patients.

The gene therapy work which has been done to date works with the patient's own stem cells and, even when successful, takes at least 12 to 18 months to halt the disease. Because so many ALD families experience their first diagnosis after the disease has started to progress, their children do not have the luxury of 18 months of time before a successful therapy may be given a chance - they need a therapy which begins to work much more quickly in order to save these children. To address this critical need, we will work with physicians and scientists to investigate using a different gene therapy approach which targets brain cells and which can go into the brain immediately and stop the disease in a much shorter time frame. In order to progress this approach, we have to test this theory in laboratory mice, and our aim is to have this initiative launched in mice by year end. If this therapy proves to be successful in mice, we will immediately initiate the additional pre-clinical steps necessary before the therapy would be ready to be tested in humans. 

Looking to our second goal, in order to properly explore whether various anti-oxidant compounds may be effective in treating symptoms of AMN, the compounds will similarly need to be tested first in mice. Toward that end, and because AMN mice need to age before they exhibit AMN symptoms, we will work with a lab to begin breeding and aging the mice as the therapeutic agents are investigated and the approach is formally defined. Our goals is to launch these activities by year end as well.

As we pursue these two near-term primary goals, we of course will continue to support and work with others to find additional promising therapeutic approaches to ALD and AMN, to implement newborn screening in as many states as will permit it, to find ways to accelerate ongoing initiatives managed by others working in this field, and to continue to help families faced with managing ALD and AMN patients.

February 2011 Stop ALD Foundation 2010 Accomplishments and 2011 Plans

The Stop ALD Foundation (SALD) had a very busy and productive 2010 which is reflected in the advances we wish to share with you as we pursue our ongoing mission to improve therapies and develop cures for adrenoleukodystrophy (ALD). Tragically, ALD continues to be a dreadful diagnosis, often accompanied by a fatal course in young boys and severe debilitation in adult men. We are working hard to create the possibility of a different result.


I'd like to share some of our highlights from 2010.

In terms of the Foundation's long-term commitment to progressing gene therapy in ALD, we previously reported on the landmark scientific paper published late last year

by our partners in this novel clinical trial. The clinical trial continues, and we are pleased to share that two more children have been treated! At the annual American Society of Cell & Gene Therapy meeting, which was held in Washington, D.C. this past May, Dr. Nathalie Cartier reported updated results in three boys. These children were selected for gene therapy because they had no donor match and were otherwise facing decline leading to death. At the end of her very well received presentation she publicly acknowledged the important role that SALD has played in this project.

We should all feel grateful that these boys, whose prognoses were uniformly fatal, are all back with their families and attending school. Our donors and supporters played a crucial role in rescuing these children. Savor this. It isn't every day that one can literally save a life.

The gene therapy program continues to grow and the academic success has been leveraged into the biotechnology industry. SALD actively advocates on behalf of this initiative with regulatory agencies. In December of 2010, the Foundation attended a series of FDA Advisory Committee hearings where issues critical to our mission were being reviewed. We are fortunate that members of this expert committee are well aware of the ALD gene therapy strategy, and our interactions with them have uniformly been positive.

Our Foundation has also been active in advancing research into treatments foradrenomyeloneuropathy (AMN). We have co-sponsored a clinical trial based in The Netherlands in which men afflicted with AMN were enrolled and treated with bezofibrate, a medicine which has been approved and used in treating disorders other than ALD and AMN. This drug is being administered in increasing doses and the subsequent levels of VLCFAs will be measured. This study has moved forward with extreme efficiency thanks to the dedication of the Dutch team of investigators. We are pleased that when they encountered promising data in the lab they approached us for financial sponsorship. Our thorough, yet fully vetted and streamlined process was able to negotiate terms quickly. We look forward to receiving top line data in the first months of 2011. If the results are promising the Dutch medical system will take over sponsorship of a much larger multi million dollar trial.

In addition to the bezofibrate study, SALD has also been active in assembling an international team that will address AMN from another angle. Preliminary investigations into the effect of oxidative stress have been met with promising results in mouse models. Following oral therapy affected mice have demonstrated marked improvement in tests evaluating their motor skills. We would like to follow up on these laboratory data with a trial designed for patients. This will require a coordinated multidisciplinary effort and a development paradigm which is SALD's specialty.

These exciting developments are just but a few of the highlights of 2010. Due to your continued and steady support, SALD has been able to be a key contributor to several innovative and worthy efforts. This has resulted in medical interventions which would not otherwise be possible. In the case of the three boys who received gene therapy, their families would now be facing terminal and end-of-life decisions for their beloved children, rather than celebrating the encouraging outcomes which they are now experiencing. We hope the same will hold true for the fourth boy who was recently treated, but for whom it is still too early to know results.

Leading The Stop ALD Foundation is a profound privilege and a great responsibility that continues to motivate me deeply on a daily basis. I reflect that my nephew, Oliver, of beloved memory, would have been starting college this year if not for ALD. I do not want any more children to be robbed of their futures. I do not want any more parents to helplessly witness the decline of their sons.

Our important work continues thanks to your dedicated and generous support. Due to the rarity of this disease and the current economic climate, it is only too easy for the desperate needs of those suffering from ALD to be ignored. The affected families who are relying on us, however, have no options. Let us not let them down.

Greetings!

What great news we were able to deliver in 2009!  While working hard in many areas, we are, by far, most proud of our groundbreaking international success in the field of gene therapy.  This is the first time ever that a fatal brain disease has been successfully treated by gene therapy, and it was ALD!  Our published gene therapy success made news around the world in the scientific and mainstream press.  In case you have not seen the details, we'll give a recap below.  With your generosity and our hard work, we pulled off what most foundations only dream of -- delivering a safe and effective therapy for a disease. There is still critically important work ahead for 2010, and we'll tell you about our goals and how we would greatly appreciate your continued support. 

Gene therapy breakthrough!

[Science on the gene therapy for ALD article] On November 5, 2009, the two French researchers with whom we have worked closely had their pioneering findings published in Science.  In this groundbreaking paper, they describe two boys with ALD who received gene therapy treatment and consequently had their brain disease progression halted, and some damage was partially reversed.

This biomedical advance is significant beyond the realm of ALD.  We would like to extend our heartfelt thanks and gratitude to you, our supporters and generous donors.  By delivering this new treatment, you have helped to make a difference in the lives of children and adults suffering from ALD and AMN globally. In addition, the approaches utilized are applicable in the treatment of many other diseases, such as metachromatic leukodystrophy (MLD), Wiskott-Aldrich Syndrome (WAS), Hurler Syndrome, Krabbe disease, and more. You should be VERY proud.

Goals for 2010

While we are pleased with our achievements to date, this success has only served to motivate us to expand and refine the next generation of diagnostics and therapies.   The knowledge and data acquired to this point have provided a solid and scientifically reliable groundwork for launching and focusing our next series of efforts.
Our goals for 2010 include:

Leverage ongoing efforts to drive forward newborn screening for ALD.Apply the gene therapy approach to a broader population, e.g. adults.Optimize the gene therapy approach to minimize use of toxic medications during the gene therapy procedure.
Drive approaches providing options for advanced-stage ALD boys who are diagnosed too late to benefit from a stem cell transplant or gene therapy.  Several cell therapies and drug options are being considered.Drive approaches to provide options for sufferers of AMN.  Animal models are being used to examine the application of antioxidants.Continue to support families who are stricken by ALD and need help navigating the way forward.

December 2007 Stop ALD Foundation Update

Highlighted in the movie Lorenzo's Oil, ALD is a terrible, often fatal, degenerative brain disease which affects 1 in 15,000 young boys. Back in 2000, few resources were dedicated towards aggressively finding a treatment for ALD. We started The Stop ALD Foundation to make a difference. Thanks to past donor support, our entrepreneurial drive, and personal passion, The Stop ALD Foundation has helped pioneer cutting-edge medical research responsible for significant advances not only for children with ALD, but also potentially for children and adults suffering from a host of genetic diseases. Exciting breaking scientific news was announced last month in Europe on a novel gene therapy clinical trial which was based on The Stop ALD Foundation's direct involvement and co-funding. A team of researchers in France used gene therapy to treat two boys deteriorating from ALD who had no other therapeutic options. While still early, the two boys are producing the previously deficient key ALD protein. This treatment could have broad implications for a number of disorders from sickle cell anemia to leukemia. This news was reported in many media outlets, including Nature.

Our major goals at this time are continuing the current European gene therapy trial and launching a parallel collaborative trial in the U.S. Additionally, The Stop ALD Foundation is supporting and facilitating research in mesenchymal stem cell therapy, identifying new therapeutic targets, advancing diagnostics, prognostics, better understanding disease pathology, neonatal testing, and helping to promote collaboration of scientists and ALD researchers in the U.S. and around the world.

As we look towards 2008, we are energized by our recent scientific and clinical developments and remain committed to driving forward the development of ALD therapies on an aggressive timetable. Doing so requires not just our commitment of time and energy, but resources as well. By giving, you can help us drive research forward that will help children suffering from ALD and, hopefully, other diseases as well. To save one life is as if you saved the world.

Winter 2004/2005 Stop ALD Foundation Update

Update Dedication

The Stop ALD Foundation was started immediately following the year 2000 ALD diagnosis of Oliver Abraham Lapin -- at the time a sweet, caring, and extremely intelligent eight-year-old boy from Houston, Texas. Oliver was misdiagnosed for years and, by the time an accurate diagnosis was made, he was already severely affected with no treatment available to help him. The correct diagnosis did provide early warning for the two other affected boys in the family who could be "saved." The expression "it takes a village" was taken to heart when Oliver's extended family started The Stop ALD Foundation in early 2001.

On Thursday, August 12, 2004, the day before Oliver's 12th birthday, Oliver passed away from complications related to ALD. He was at home lovingly surrounded by his family.

The Stop ALD Foundation wishes to thank the out-pouring of support and donations that we have received in memory of Oliver. We will continue with our mission to make sure more children are diagnosed earlier, and to facilitate the availability of better, more effective, and safer treatments.

We'll so deeply miss you Oliver. We'll never, ever forget you.


We continue to make progress and leverage our funding and resources to amplify and magnify dramatically the donations from the generous people and organizations that have funded the Foundation to date. We continue to need your financial support to drive forward our efforts to save the children and adults devastated by this horrific and rare disease.

The Foundation continues to oversee and participate in advancing five primary projects:

1.     Gene TherapySince stem cell transplants (e.g., cord blood transplantation and bone marrow transplantation) are the only known therapy for treating ALD, gene therapy is designed to take a stem cell transplant a step further by finding a safer and universally available transplant for all ALD patients.

2.      ALDR Up-Regulation: Aims to find a drug compound (medication) that will provide a therapy.

3.      Predicting ALD onset and finding new pharmaceutical compounds using Transcriptomics: Transcriptomics is the description and study of gene expression patterns. This work is aimed at learning more about the disease to open additional avenues of treatment and be able to predict the expected impact of ALD on a particular child.

4.      Mesynchymal Stem Cells (MSCs): Currently transplants are only effective if done early enough in the disease progression. MSCs may be used to provide a treatment for later stage patients.

5.      Awareness and Prevention: We have directly influenced 15,000 US physicians with a world-class professional information pamphlet on ALD in order to increase the early diagnosis of ALD.

Therapy Development

Gene Therapy

Gene therapy research and implementation as a treatment for ALD has been the highest priority since the inception of The Stop ALD Foundation.  In the early days of The Stop ALD Foundation, a thorough review by ALD experts, along with an international multi-disciplinary team, concluded that gene therapy applied to the patients own stem cells was the best approach to pursue.

The following steps have been completed:

·       Comprehensive preclinical lab experiments have yielded encouraging results, which have allowed the approach to continue moving forward. This includes work in tissue/cell cultures and in rodents.

·       The design of the gene therapy vector itself (the product which will enter an ALD patients stem cells and insert the corrected sequence of genetic material (DNA) that codes for a proper ALD gene) has been through numerous modifications, enhancements and follow-up experimentation. Because these are new biological agents, each of the reconfigurations is a result of new ideas and research applications that have never been attempted before.

·       The actual gene therapy vector has recently been manufactured by a California-based biotech company. It is the very first time this particular vector will be used in humans. This process has been both expensive and time consuming given the stringent regulations in place in both the US and Europe that must be applied to novel biologicals such as this custom vector. There has been a high degree of expertise, resources, time, and funding (approximately $1 million) that have been expended on the vector itself.

·       The French equivalent of the USs FDA is known as AFSSAPS. Gene therapy clinical trials must be approved by AFSSAPS prior to opening for enrollment. A comprehensive application (consisting of hundreds of pages -- many of which were translated into French) was submitted to this regulatory agency several months ago. The feedback was quite encouraging and only a few small details remain to be resolved between the agency and the research team.

Given that most of the preclinical work has been performed in France, the principal investigator, Dr. Patrick Aubourg and his team (all based in Paris) will be evaluating the clinical trial participants on a regular basis. Given the high expense involved in enrolling, treating, and closely monitoring patients, we are pleased that the French INSERM (similar to the NIH in the United States) will be financing this portion of the trial (approximately $1 million).

We are very pleased to announce that we fully expect this gene therapy protocol to be approved and open for patient enrollment in France in early 2005.

The objective of this initial trial will be to test both the safety of this new approach, as well its effectiveness. Usually, in an initial trial such as this, the regulatory bodies such as the US FDA or Frances AFSSAPS will only allow initial testing of safety (to make sure the therapy does no harm). However, since ALD is a rare disease, the trial will most likely be allowed to study how effective the treatment is as well. This is both unusual and wonderful news since the research team can learn more from the initial trial.

The initial trial will consist of only 6 patients. Most likely these boys will reside close to Paris. From a disease perspective these will be children who are considered good candidates for stem cell transplant (relatively earlier stages of cerebral ALD), but do not have good matches available. These boys would otherwise have to wait for a match to become available all the time getting sicker and sicker, or they might possibly be transplanted with a significantly mismatched donor and need to face the sometimes fatal results of this type of procedure.

The ALD gene therapy project is a multimillion dollar protocol. The Foundation was successful in spending only a fraction of this amount in order to marshal these resources on behalf of ALD research.

With direct Foundation expenditures of less than $200,000 we have received capital and other investments of greater than $2 million (a 10x boost to our seed investment).

ALDR Up-Regulation:
A Gene Very Similar to The ALD Gene

The second project is to pursue ALD homologue up-regulation. "ALD homologue up-regulation" is a way to encourage a gene that already exists (in design and function), but that exists in its normal form in all ALD patients. The desired effect would be for this normal gene to "over-express" so that it could compensate for the initial ALD defect. Work in the mouse model has led us to believe this could have actual impact on disease progression in ALD patients.

The Stop ALD Foundation has arranged for GlaxoSmithKline (GSK), the worlds second largest pharmaceutical company, to share a small part of their library of compounds with the Austrian lab that is the leader in this category of ALD research. A series of experiments have been designed and promising results have begun to emerge.

Data was presented at a recent ALD conference in Paris. The systematic, small molecule approach that has been taken in this latest research has generated interest and optimism from several professional groups. The Foundation is now in discussions to transfer this European project over to a German group who are looking into gaining the participation of a reputable and well-established Swiss biotech.

Again, as a result of The Foundation "seeding" this work by investing a relatively small amount of resources, we are hopeful this project will now be pursued more aggressively. We will monitor progress in Europe, and we intend to participate as further needs may arise.

Transcriptomics: Predicting an ALD Childs Fate and Finding New Pharmaceutical Compounds to Help

The third project, Transcriptomics, is aimed toward learning more about the various forms of ALD. This may offer two broad categories of opportunities. First, once pathways and mechanisms are better described and understood, specific targets can then be identified where pharmaceutical intervention may prevent or ameliorate clinical signs.

Another area of interest is prediction of disease phenotype (how the disease "presents itself" in a particular child). Given todays therapies, there would be tremendous value in predicting whether a particular young boy was facing cerebral involvement in the near future, or whether he was destined to suffer from AMN as an adult.

Current stem cell therapies (both bone marrow and cord blood transplantation) are very risky procedures and often, the outcome is better if performed when the patient is younger. If the family and their doctors could have a definitive technique to predict whether a young child was going to have a deadly cerebral onset as a child, that technique would be an extremely useful tool when making the decision if and/or when to transplant a particular patient.

We must remind ourselves that we still do not fully understand the pathology of this dreadful disease. Many questions still exist: What role (if any) is played by the elevated long chain fatty acids? Why do males who carry identical gene mutations have different forms of ALD? Why do some female carriers show AMN symptoms (e.g., walking and bladder control problems) and others dont? Can the form of disease presentation be predicted? The answer to all these questions, and many more, is, unfortunately, at the present time, "we dont know."

In an effort to better understand the mechanism of the disease, The Foundation has arranged for an interactive and cooperative Transcriptomics project between GlaxoSmithKline (GSK) and Dr. Patrick Aubourg (INSERM, Paris, France). Transcriptomics is the description and study of gene expression patterns.

Most fortunately, we have been able to utilize hundreds of thousands of dollars worth of equipment and expertise in order to better describe and understand the complex protein production and interactions that occur in the various human ALD pathological states.

Furthermore some of this data is being analyzed via very sophisticated proprietary techniques so that useful interpretations can be made. The next steps with this technology will be to try to develop a better prediction technique for patients.

Mesynchymal Stem Cell (MSC) Therapy

The Mesynchymal Stem Cell (MSC) Therapy project involves scientists and physicians from St Judes Hospital (Memphis, Tennessee), Childrens Hospital of Philadelphia, Tulane University School of Medicine (New Orleans, Louisiana), and leading researchers and physicians in Germany. This experimental therapy involves the use of MSCs taken from the bone marrow of adult donors. The MSCs would be delivered into the blood and brains of ALD patients who are at an advanced ALD stage. In the first phase trial of MSCs, these stem cells would be used in conjunction with conventional bone marrow transplants.

This work will be ongoing in 2005 and we look forward to reporting on the early research results.

AWARENESS & PREVENTION

ALD MediView Report Sent To 15,000+ US Physicians: Dont Misdiagnose an ALD Child as Having Attention Deficit Disorder (ADD)

Our fourth area of focus relates to prevention. This past spring The Foundation secured the cooperation and participation of a medical information company, Rogers Medical Intelligence Services, which distributes continuing education to physicians. On the Foundations behalf, Rogers engaged a professional medical technical writer to research and write a comprehensive report detailing the medical presentation of ALD along with information regarding diagnosis, common misdiagnosis, treatment and prognosis.

Unfortunately, every year young boys die from ALD merely because it took too long for them to be diagnosed accurately. Typically these children are misdiagnosed as having attention deficit disorder (ADD) or attention-deficit/hyperactivity disorder (AD/HD). Thus, we targeted 15,000 US physicians who have the highest likelihood of seeing these boys in their practices. It is essential that we heighten awareness among doctors so they will be aware of ALD and help to diagnose ALD accurately while there is still time to help the children.

CHANGES COMING TO THE STOP ALD FOUNDATION'S WEB SITE

New Tools to Help Families Desperately Seeking Accurate Information, Direction, and Answers

In early 2005 on StopALD.org there will be a new and very comprehensive Frequently Asked Questions (FAQs) section. In this section well include the answers to the most commonly asked questions that we receive.Monthly, we receive and quickly respond to hundreds of emails from families, researchers, and patients all over the world. Many of them have the same questions. We have been compiling our responses and plan to post them to this new Frequently Asked Questions (FAQs) section of the web site.

Another change coming to the site is a section entitled "I just received a diagnosis of ALD. What do I do?Many inquiries we receive are from families who have just received the absolutely horrifying and devastating news that a loved one has been diagnosed with ALD. They are scared, confused, and lack accurate, credible, and timely information about what they need to do immediately. ALD can move extremely fast along its destructive path. Sometimes, when the diagnosis is received, its an early diagnosis of a younger sibling to a related older child who has been diagnosed with ALD, and now the younger child is found to have the disease as well, but is not yet symptomatic. More often then not, unfortunately, the diagnosis is received and the child is already affected. At this point, sometimes, an emergency intervention can save the childs lifeTime is of the essence. Sometimes, its too late and there is nothing that can be done to save the child. But, sometimes there is a very short "therapeutic window" measured in days or weeks where parents must move extremely fast in order to even have a chance at saving a childs life.

In the "I just received a diagnosis of ALD. What do I do?" section of the web site, well include:

·       check list of detailed questions to ask medical professionals (along with a brief explanation as to the meaning and importance of the questions). We have found that most people dont know the critically important questions they need to start asking immediately to ensure they are getting the best possible advice and care.

·       Steps that must be taken immediately for children/adults with ALD/AMN and family members (e.g., having other children and family members tested).

·       Common ALD-related terms as there is a whole new set of words parents and care givers will need to understand.

UPDATE OVERVIEW

This update details how The Stop ALD Foundation is successfully continuing to take an entrepreneurial approach to Adrenoleukodystrophy (ALD) and Adrenomyeloneuropathy (AMN). In this update we will cover how we continue to drive forward on our three main focus areas:

THERAPY DEVELOPMENT

Gene Therapy

Gene therapy has been a top priority for The Stop ALD Foundation since its inception. Since the discovery and description of Adrenoleukodystrophy in 1910, a comprehensive understanding of the disease process has eluded scientists. At the dawn of the new millennium, the role of very long chain fatty acids (VLCFAs), the metabolite that is abnormally high in individuals with a defective ALD gene, has yet to be determined. The reason one patient gets one form of the disease at a particular age versus another patient getting an entirely different form is completely unknown. While The Foundation fully supports furthering knowledge in these essential areas, searching for therapies the only way to help patients is The Foundation's top priority.

One important piece of the puzzle that has definitively and uniformly been determined is that all patients afflicted with ALD have a defect in the ALD gene. Therefore, in an effort to take advantage of the recent explosion of knowledge and technology in molecular biology and genetics, the simple yet elegant notion has arisen that would employ inserting a correct copy of the ALD gene DNA which codes for the ALD protein. Early work performed both in cell cultures and in mice has shown that this approach demonstrates meaningful promise as a potential therapy for patients.

The last Stop ALD Foundation update reported that the Foundation had secured the partnership of the biotechnology company, Cell Genesys, which is headquartered in California. While Cell Genesys made huge advances in this project, the company had to respond to the economic climate by narrowing its therapeutic focus during the first quarter of 2003. While this is certainly disappointing news, The Foundation is now in negotiations with the company as well as with various leading investigators in the U.S. and Europe to establish a new series of partnerships. The Foundation will be able to take advantage of much of the hundreds of thousands of dollars of investment already made by Cell Genesys on behalf of this project; it is simply a matter of filling in the gaps in the most efficient and productive ways possible.

Part of the reason Cell Genesys shifted its focus was the perceived climate shift that occurred in response to an adverse event encountered in a French SCID (bubble boy) gene therapy trial. Although the vast majority of treated children were effectively cured from this often-fatal disease, two of these boys now are battling leukemia, so far successfully.

In immediate response to this issue, The Foundation took a very proactive stance. Foundation action included a private meeting with the director of the division in the Food and Drug Administration (FDA) responsible for gene and cellular therapy. In that meeting, The Foundation stressed the importance of advancing the gene therapy agenda in diseases, such as ALD, which are so devastating. The meeting was extremely positive and productive and The Foundation firmly believes that the FDA is receptive to innovative therapies such as gene and cell therapy for deadly and rare diseases like ALD.

Multiple representatives from The Stop ALD Foundation attended and participated in several FDA and National Institutes of Health (NIH) meetings held in Washington, DC to ensure that progress in gene therapy continues. The meetings and participation worked. The FDA allowed research to continue.

The Stop ALD Foundation's gene therapy project is ongoing. Critical pre-clinical work is being conducted at laboratories in France and the U.S., all of which is necessary prior to opening the clinical trial for humans. As a result of the Foundation's work, this project continues to receive very strong encouragement from many sectors, including the FDA, NIH, the French INSERM (the equivalent of the U.S. National Institutes of Health), AFM (a very influential and well funded group that is the French equivalent of the Muscular Dystrophy Association) and leading ALD research institutions in the U.S and France. We have also been fortunate in securing the participation of the leading investigator from the French SCID trial, a dedicated researcher of international acclaim.

To ensure that the Foundation can get to human trials as quickly as possible, during this past American Society of Hematology meeting (ASH) in December 2002, The Foundation's Chief Science Officer secured the collaborative support of a research team that has one of the very few groups of monkeys that have been transplanted with cells containing the gene therapy vector quite similar to the one The Foundation proposes to use in the ALD clinical trial.  This monkey colony was created for other research purposes.  In clinical trials, the issue of safety comes up as well as efficacy.  And quite naturally, people are often quite interested in results obtained in larger animals than mice. Non-human primates fit this interest.  This is a reasonable question for the anticipated ALD gene therapy trial.

When the opportunity arises to study central nervous system tissue in these monkeys, The Foundation will help to ensure that the analysis takes place.  When the original monkey experiment was conceived these scientists weren't interested in brains, let alone ALD. Now The Foundation has made a major inroad into this path of animal subjects animal subjects that are extremely costly and thus very limited in number and hard to come by. Once again, the Foundation has leveraged your funding and gotten another already funded research team interested in ALD and willing to help us to find a cure for ALD.

Mesynchymal Stem Cell (MSC) Therapy

The Mesynchymal Stem Cell (MSC) Therapy project involves scientists and physicians from St Jude's Hospital (Memphis, Tennessee), Children's Hospital of Philadelphia, Tulane (New Orleans, Louisiana), and leading researchers and physicians in Germany. This experimental therapy involves the use of MSCs taken from the bone marrow of adult donors. The MSCs would be delivered into the blood and brains of ALD patients who are at an advanced ALD stage. In the first phase trial of MSCs, these stem cells would be used in conjunction with conventional bone marrow transplants.

The theory is that these MSCs can start halting and repairing disease damage rapidly while the full bone marrow transplant takes effect. These patients are not suitable candidates for traditional bone marrow transplants due to the advanced state of their disease. These are advanced-stage ALD patients who are running out of time and cannot afford the typical 6 - 18 months it takes for a bone marrow or cord blood transplant to halt ALD disease progression. This time lag would either lead to very severe physical and mental debilitation (i.e. rapid progression to a vegetative state) or death. This unique therapeutic endeavor involving MSCs would allow for rapid and immediate delivery of donor cells to the brain the ultimate destination of importance in an ALD patient, with the hope that the positive impact on the disease can thus begin much sooner. Meanwhile, the traditional bone marrow or cord blood transplant is undergone in conjunction with the experimental therapy in order to take advantage of the well-recognized longer-term benefits of these types of transplants. The MSCs would come from the same donor as the bone marrow used for a bone marrow transplant.

In December 2002, in conjunction with the American Society of Hematology (ASH) Conference in Philadelphia, The Stop ALD Foundation hosted a workshop for all key MSC therapy participants in order to review the project. Many details were discussed and everyone agreed on what they need to do to move forward. Today, the work is moving forward in creating the MSC Phase I clinical trial protocol documents and we expect that it will be submitted to the FDA in 2004.

Up-Regulation of ALDR:
A Gene Very Similar to The ALD Gene

The human genome was sequenced, almost in its entirety, some three years ago. The successful sequencing produced myriad findings about human DNA. One is that some human genes overlap in their structure with others. It is almost as if some genes are incomplete or slightly inaccurate tracings of others. These genes are termed homologues.

Recent evidence suggests that the gene most homologous to the ALD gene (which is also known as ABCD1) may have a somewhat overlapping function. This homologous gene is called ALDR or ABCD2.

The protein of the ALD gene is either nonfunctional or not produced in someone with ALD; however, that person's homologous genes, such as ABCD2, function properly. That being said, these homologous genes are only producing at normal capacity which is not enough to make up for the original ALD genetic defect. Recent laboratory experiments have demonstrated that if one were to over express the ABCD2 gene so that there was a super-normal level of that gene's activity, it may have an impact on the course of ALD. When a gene produces a higher level of protein, which is the same as over expressing the protein for which it codes, this is termed up-regulation.

So the question arises: How can the ABCD2 gene be manipulated to be up-regulated in a safe yet effective manner? Researchers know that different drugs have direct impact on the activities of various genes. In both the public and private domain there are thousands of compounds that have only been examined in a narrow fashion. The drugs that are being investigated or even being currently used to treat, for example, acid reflux, are not also analyzed to see if they have a specific impact on any one particular gene out of the 35,000 genes. This is called cross-use. One well known example of cross-use of a drug is Viagra. This compound was originally examined for cardiovascular purposes. Its effect on male impotence was initially regarded as a side effect. Ultimately this effect became the primary indication for use of the drug.

It is highly possible that given the incredibly vast quantity of pharmaceutical compounds that have been both discovered and created over the last 100+ years, that drugs may already exist which may have the ability to up-regulate a particular gene of choice. In the case of ALD, one gene of choice would be the homologue referred to earlier  ALDR. This research entails very sophisticated and very costly screening methods. In this effort, The Stop ALD Foundation has gained the cooperation and support of one of the worlds leading pharmaceutical companies, GlaxoSmithKline (GSK). After numerous meetings in the US and Europe, as well as discussions between The Foundation, GSK, and leading researchers in the field, an organized methodology was agreed upon. Certain compounds will be evaluated in a leading laboratory in Vienna, Austria headed by a world-renowned scientist, Dr. Johannes Berger, while another category of compounds will be further investigated by a leading research institute in Strasbourg France headed up by another internationally acclaimed researcher, Dr. Jean Louis Mandel. Since these are all laboratory experiments, The Foundation expects that data will begin to emerge over the next few months that will help to further refine and target ongoing research. This is a classic Stop ALD Foundation strategy: put all the potential contributing parties together, ultimately resulting in an efficient and directed series of scientific collaborations.

Adrenomyeloneuropathy (AMN):
Following All Leads

The Stop ALD Foundation continues to investigate potential therapies for Adrenomyeloneuropathy (AMN). AMN is the name used when ALD manifests itself in adulthood. While the Foundation is not currently involved in any AMN clinical trials, the Foundation actively discusses this form of the disease at all scientific meetings it attends always looking for new ideas and potential therapies. We are in contact with leading AMN researchers in the US and Europe and are following up on their trials and seeing how we can assist them in moving forward.

AWARENESS

Umbilical Cord Blood for Transplant in ALD Patients

Umbilical cord blood used in transplantation already has a proven track record of saving lives. The Stop ALD Foundation continues to educate patients, family members, and physicians concerning the pros and cons of cord blood. Sadly, many physicians are unaware of the effectiveness and viability of umbilical cord blood transplantation as a therapy for ALD patients who are not yet severely affected. Umbilical cord blood should be considered as an option for certain candidates. In support of cord blood as a stem cell transplantation option, members of The Stop ALD Foundation have testified before the US Senate and at FDA advisory meetings, concerning a new bill being introduced that would provide funding for a US National Cord Blood Bank Network. This funding and network would increase and sustain the number of cord blood units available for transplant in diseases that would include ALD. Thus, no patient will be declined and delayed having a transplant due to a lack of suitable donor cells. Unfortunately, today, this is an ongoing reality with tragic consequences.

PREVENTION

Screening Newborn Babies for ALD

Today every state in the US has mandated newborn screening for all newborns, however, that mandate can be for screening for as few as two diseases (Phenylketonuria (PKU) and Congenital Hypothyroidism). A few states mandate screening for up to 25 diseases. Mandated testing is on a state-by-state basis. Neo Gen Screening, Inc. (www.neogenscreening.com, phone 866-4-NEOGEN, from outside the US call +1.412.220.2300) currently offers a reasonably priced supplemental newborn screening test (less then $100) that screens newborn babies for dozens of rare diseases. Currently, that test does not include screening for Adrenoleukodystrophy (ALD). Neo Gen Screening is refining new technology that would enable ALD to be included in the test. Inclusion of an ALD test would raise the cost of the test by only one to two dollars. Neo Gen Screening is hopeful that their new newborn ALD test will be validated and ready for market by the end of 2004.

Many people mistakenly think that all newborn babies are screened for a large variety of rare diseases where early detection can save lives. Unfortunately, due to the way that the U.S. healthcare system works, the comprehensiveness of such testing (and the diseases screened for) varies widely, and families are often completely unaware of the level of screening performed on their newborn. Part of the reason that such testing is not comprehensive is that the cost involved is an expense which is not immediately recouped by the hospital or medical center where the baby is born. Even though insurance might pay for the bulk of the expenses related to having a baby, such expenses at one institution could include a post-natal screening for 45 diseases while at another institution they may only include a screen for two or three diseases. In spite of this, comprehensive post-natal testing is not even offered to new parents as an option for which parents would have to pay a small additional fee (less then $100).

Currently, if parents want to be certain that their newborn baby is screened for a wide variety of diseases, they must first check with the hospital where the child was born about what screens have already been performed and then contact a company such as Neo Gen Screening to begin the supplemental testing process. The testing company will send out a testing kit that parents take to their pediatrician. A small blood sample is taken from the baby and the kit is sent to the testing company for processing. Parents then get the results from the company. The cost for this supplemental testing is around $60 which includes the testing kit and processing fee. As well, there would be charges from the medical professional for drawing the blood sample.

Comprehensive post-natal testing is highly recommended because of the knowledge gained about the possibility of a rare disease surfacing in a newborn baby's life. For many of these diseases, including ALD, early detection, before clinical signs appear, can dramatically improve the chances of successful therapeutic intervention.

Screening for ALD is not mandated by any state, and the prospect that an ALD screen will be included in a post-natal test does not necessarily make it any more likely that such a test will be required by states.

The Stop ALD Foundation has initiated a dialogue with Dr. Michael Watson, Executive Director of the American College of Medical Genetics (www.acmg.net). Dr. Watson currently heads a federal committee of genetics experts mandated to advise the US government on neonatal screening for a wide variety of disorders. The Foundation's goal is to have ALD included in the list of diseases for which all newborns are screened. Thus, once the screening technology is in place, families and their children will not have to suffer the tragic consequences of facing an unsuccessful battle with ALD strictly because the disease was diagnosed too late. To help reach this goal of making sure that all newborn babies are screened for ALD, the Foundation made a highly scientific and very detailed submission(www.acmg.net/surveys/NBS-05_22_03/nbs.asp) to Dr. Watson which will be incorporated into the development of a critical decision making tool. We fully expect our contribution to have significant impact on the decisions and recommendations made by the committee's appointed working group.

The Stop ALD Foundation is in regular contact with Neo Gen Screening and reviews progress with the scientists assigned to completing the ALD test. In an effort to collect samples that will be utilized in validating and fine tuning the test, Neo Gen Screening would like to receive anonymous samples from newborns with or without ALD. This will help Neo Gen Screening test the accuracy of their new test in order to help others. It is important to note that test results collected during this test validation phase will be processed in a completely anonymous fashion. No names or identifying information will be utilized. If you elect to assist by providing a blood sample, you will be helping a new test come to market that will help to catch ALD at birth.

CHANGES COMING TO THE STOP ALD FOUNDATION'S WEB SITE

New Tools to Help Families Desperately Seeking Accurate Information, Direction, and Answers

There are a number of changes that will be coming to the web site (www.stopald.org) by the end of this year. One change will be the inclusion of a detailed Frequently Asked Questions (FAQs) section. In this section we'll put the answers that we provide to the most commonly asked questions that we receive. Monthly, we receive and quickly respond to hundreds of emails from families, researchers, and patients all over the world. Many of them have the same questions. We have been compiling our responses and plan to post them to this new Frequently Asked Questions (FAQs) section of the web site.

Another change coming to the site is a section entitled I just received a diagnosis of ALD. What do I do? Many inquiries we receive are from families that have just received the absolutely horrifying and devastating news that a loved one has been diagnosed with ALD. They are scared, confused, and lack accurate, credible, and timely information on what they need to do immediately. ALD can move extremely quickly along its destructive path. Sometimes, when the diagnosis is received, it's an early diagnosis as a related older child has been diagnosed and now a younger child is found to have the disease as well, but is not yet symptomatic. More often then not, unfortunately, the diagnosis is received and the child is already affected. At this point, sometimes, an emergency intervention can save the child's lifeTime is of the essence. But for ALD patients and their families, there is a tremendous lack of accurate, up-to-date information in the medical community. Sometimes, it's too late and there is nothing that can be done to save the child. But, then again, sometimes there is a very short therapeutic window measured in days or weeks where parents must move extremely swiftly in order to even have a chance at saving that child's life.

We have found that most people don't know the critically important questions they need to start asking immediately to ensure they are getting the best possible advice and care.

April 2002 Update

In this first update on the activities and progress of The Stop ALD Foundation (SALD), we want to first and foremost thank you for your participation in the fight to seek out and develop promising therapies to prevent, diagnose, and cure adrenoleukodystrophy (ALD). SALD's guiding principal is to make investments using your generous donations that will most effectively lead to viable therapies in the shortest time frame. Because of the rarity of and lack of attention paid to ALD, even when compared to diseases with similar levels of incidence, research money devoted to fighting the disease is comparatively miniscule. Thus, every dollar SALD strategically deploys in the fight can really make a difference. We want to update you on SALD's progress to date.

SALD ONGOING PROJECTS IN CONNECTION WITH TREATMENT OPTIONS

Treatment options for ALD sufferers are extremely limited. The only uniformly accepted and recommended therapy is bone marrow transplant (BMT), but the procedure itself is risky and not even an option for all sufferers of the disease. In fact, BMT is only suitable during a relatively narrow window of opportunity before ALD has progressed too far. As BMT does not generally reverse ALD, which continues to progress for six to 12 months even after a successful procedure, lack of an early diagnosis of the disease can mean that a patient will not benefit from the procedure. BMT may also not be appropriate for some patients because there is no good bone marrow match found. Additional therapies must be developed to fight ALD.

1. GENE THERAPY

One of the promising new therapies, which SALD is helping to develop, is gene therapy. Several members of SALD met with Dr. Patrick Aubourg, a researcher in France who is the only doctor in the world developing gene therapy for ALD. Dr. Aubourg has published in The New England Journal of Medicine, Nature, Lancet and other top-tier journals. His published work concerns the diagnosis, therapy and understanding of ALD disease mechanisms. Dr. Aubourg has been doing important research related to ALD gene therapy for over eight years, but his promising work has been slowed by the lack of qualified personnel and resources devoted to it. In keeping with SALD's policy of channeling resources to promising therapies in development, SALD saw an opportunity to enhance and accelerate Dr. Aubourg's research. To that end, SALD has sponsored a tremendously skilled and experienced post-doctoral student to work in Dr. Aubourg's lab. She is already running various experiments in the lab in Paris in preparation for a human clinical trial.

To carry out the human clinical trial, researchers will need a large quantity of gene therapy "product" of the very highest quality. There is only one company, Cell Genesys, in the U.S. currently on track towards making the appropriate product (also called a vector) in the near future, and SALD is proud to say that the company agreed to cooperate with SALD's plans for the human trial. This development is a direct result of a series of meetings organized or implemented by SALD and held throughout the U.S. and Europe between Cell Genesys, SALD scientific advisors, various ALD researchers, FDA advisors, and SALD members. The project was officially launched by Cell Genesys on March 1, 2002. As Cell Genesys is only providing the vector, SALD itself is driving this huge but exciting project and is responsible for getting the actual clinical protocol up and running. Ongoing meetings and discussions are occurring even now to accomplish this, and SALD will commit additional resources and personnel to the project over the coming year. SALD aims to have the project protocol and plan reviewed and approved by relevant governmental advisory and regulatory committees by Q2 2003.

In another development on the gene therapy front, several members of SALD met in March 2002 with Dr. Alain Fischer in Paris to specifically discuss ALD and gene therapy. This meeting was important because Dr. Fischer is the only researcher in the world so far who has, by most counts, cured a fatal pediatric genetic disease. Everyone agrees the children he treated are now basically fine; however, it has only been a few years and so it is too early to call it a definitive cure. Dr. Fischer will be one of the key participants in the European arm of our gene therapy clinical trial.

2. CELL CLONING THERAPY

In another promising area of research, SALD is working with Advanced Cell Technology (ACT), a highly innovative company in Massachusetts working in the area of therapeutic cloning. The concept is that if someone has liver failure, for example, and a donor liver is not available, doctors could remove some cells from a part of the person's body (likely the blood, skin or from the organ in question itself) and then reprogram them to develop into the required tissue once they are returned to the person's body. The obvious advantage of therapeutic cloning is that there are no rejection problems typical with transplanted tissue or organs because the cells come from the person's own body.

A similar scenario with ALD would be as follows: cells are taken from a person with ALD, the defect causing the ALD is repaired, the cells are expanded and then reprogrammed to become the type of cells used in the one successful model for treating ALD that already exists: BMT. Therapeutic cloning sounds farfetched, but it is only a matter of refining the technology because the process was recently proven to be successful in an animal model. Members of SALD met with the CEO and other officers of ACT, and the company is committed to helping the Foundation. So far, ACT has underwritten the bulk of the expense of this project. SALD is in contact with ACT on a regular basis and both sides meet to discuss progress with the project. SALD is constantly tweaking its end of the protocol, which involves providing ACT with various ALD samples.

3. MESENCHYMAL STEM CELL THERAPY

SALD is also contributing resources to research involved with another type of cell called a mesenchymal stem cell (MSC). This cell has shown promise in terms of differentiating into various brain cells. With ALD, children get sick and die because they lose myelin, the "white matter" of their brains. Recently MSCs have shown promise in the area of remyelinating brains. On April 8, 2002, SALD conducted a meeting with the world's leading experts in MSCs (which are currently used in the treatment of some other diseases) along with ALD experts so that MSCs and their potential as an ALD therapy could be explored. The parties outlined a clinical trial that would involve giving MSCs to patients in the more advanced stages of ALD who have already elected BMT and have found a perfect bone marrow donor match. Because of the progression of the disease for a period after BMT, the hope is that the addition of MSCs to the protocol will allow for cells to get into the brain much sooner and ultimately improve the long term outcome. Much of this theory is based on preclinical data performed by researchers who attended the April 8 meeting. One of the proposed primary investigators, Dr. Ed Horwitz from St. Judes Hospital in Memphis, is one of the few hematologists in this country that already has experience giving MSCs to children with a genetic disease. He has a track record and is eminently qualified and motivated to help these "index case" patients, typically the first ones to be identified with a genetic disease in a family.

 

4. PROTEINOMICS

SALD continues to explore one of the most painful aspects of this disease, the timing of its onset. A child is diagnosed with the ALD defect in his genes. Will he develop the devastating disease as a child or will he get adult-onset ALD, itself a physically disabling illness with a 30% chance of fatal brain involvement? Through SALD's efforts, GlaxoSmithKline (GSK), the world's second largest pharmaceutical company, has initiated a project to help answer this crucial question. By using sophisticated technology (which has cost millions of dollars to develop), GSK researchers will analyze ALD tissue samples provided by SALD and determine if data can be generated to elucidate this issue. This field is called proteinomics.

So many therapeutic options for ALD have not been explored for the simple reason that the world's leading experts in areas like gene therapy, therapeutic cloning, proteinomics, and MSCs go about their fascinating work without ever encountering ALD.
